The Healthy Youth Survey includes questions related to physical activity and diet, unintentional and intentional injury, substance use, risk and protective factors, and access to school-based services. Survey results are used by schools, communities, and state agencies to plan programs to support our youth and reduce their risks. Participation in the survey is voluntary and the students’ answers are anonymous. There are no codes or information used to match a survey to a student. Each school will choose a week in October to administer the survey. Your school will inform you when your student’s survey will take place.
Information for parents and students can be found here. **Please note, 6th graders will not be asked the extra question about gender identity. Tahoma students will not be asked optional questions around sexual behaviors, abuse, or exploitation. If you would like to look at the survey questions, they are available here at the Healthy Youth Survey website. Please refer to the note above for questions not being asked of Tahoma students.
